SMS Notification Opt In Confirmation

SMS gateways are becoming very strict in the U.S., especially Twilio.

Add an optin-check box for SMS notifications so when a customer opts in the SMS reminders will be sent, but if a customer does not opt in, then the SMS notifications will not be sent.
