Document hooks (action/filter) for customisation

We are a web agency that and often create websites for clients that need just that little bit extra customization in third-party plugins. We often fall back to WordPress' hooks (action/filter) and extend our functionality in the function.php of the child theme.

Searching through the source code there are many actions and filters being used, but none of them are documented.

Would love to see some documentation happen!
