Consistency with Create New Accounts forms

It would seem that the packages and gift card create account form both use Full Name instead of first and last name. The Book Appointment uses First and Last name. It would be great to make those consistent, separating the first and last names in both the gift card and package areas also. The issues it creates are several. While it is easy to combine First and Last to make Full Name it is not possible to reliably do the other way around. This effects the user account creation, the email forms and how they are written (notifications are by default using Full Name as the string, the name in the email is blank when sent) and more. Thank you.
